Speaker

Description

Improving the health and wellbeing of your community takes a commitment well beyond the walls of our longstanding community hospitals and physician practices. As the pursuit of value-based care approaches it's "20th anniversary", there is broad recognition that health care systems simply cannot shoulder this ambitious pursuit alone. This session will explore the importance of community partnership and collaboration, describe best practices in measuring and identifying barriers, and determine actionable next steps in addressing the social and structural vulnerabilities that hinder the path to healthier populations.

Learning Objectives

Upon completion of this webinar, participants will be able to:

  1. Describe the evolution of a 20-year-old conversation on the topic of improving value in health care
  2. Measure and identify barriers to improved health outcomes
  3. Apply best practices in establishing partnerships with stakeholders and determine actionable next steps
